Daily KPI Updates

For supervisors & leads only
Please complete one Daily KPI Update per shift, per site. This form is how we track safety, staffing, and performance, so accuracy matters.

Before You Submit

  • Who fills this out? The lead or supervisor on duty for that shift.

  • How often? Once per shift, per site (1st, 2nd, 3rd). If there’s no activity for a shift, still submit a form to record that.

  • When? As close to the end of the shift as possible, after walk-throughs and checks are complete.

  • Be accurate. These KPIs are used for client reporting, audits, and staffing decisions. If you’re unsure about a question, ask your manager before submitting.

  • One form = one shift. If you cover more than one site or shift in a day, you’ll submit separate forms for each.

A. Identify
Select the site you are reporting for this shift only.
Choose the shift you are currently closing out.
B. Dock & Receiving
Are dock plates, pits, and staging areas clear of trash, wrap tails, and loose product?
Answer based on your final walk-through at the end of the shift.
Did you see any wrap tails or banding left in powered-equipment paths?
Enter the number of issues you found and fixed. Enter 0 if there were none.
Include any strap, plastic, or banding left where equipment travels.
C. Aisles & Storage
Are main travel aisles clear of trash, broken pallets, and obstructions?
Think about main routes where people or equipment move all shift.
Count any area that needed mopping or spill response. Enter 0 if none.
Were any fire exits or egress paths blocked when you checked?
Include doors, hallways, and marked exit paths.
D. Restrooms & Breakrooms
Were all scheduled restroom checks completed this shift?
Base this on your restroom checklist for the shift.
Did we run out of soap, towels, or toilet paper at any point?
Answer “Yes” even if it was fixed later in the shift.
Use the number of complaints reported to you this shift. Enter 0 if none.
E. Trash & Cardboard
Were all scheduled trash/cardboard pulls completed?
Include all areas on your route (dock, aisles, restrooms, breakrooms).
Count any piles that should have been removed before you left. Enter 0 if none.
F. Daily KPI Summary

Input 0-100% coverage.

Input 0-100% completion. Approximate is OK if you don’t have the exact number.

Use your scheduled vs. actual staffing.
100% = fully staffed, less than 100% if people were out or not replaced.
